What's this one about?  Drugs.  Plain and simple.  How drugs distort your preception of reality, breakdown the world around you until nothing is familiar, and once you've fallen this far into this pit, it's simply a matter of time.  "The sandman bleeds you of your mortality?"  Think of "sand" as "cocaine", and the "sandman" is the "drug dealer".  He tempts you with more drugs & "fuels the fire of your insanity" by presenting you with something to fuel your addiction, which is the source of your insanity.  No, I do not speak of any sort of personal experience, I'm FAR too intelligent to even CONSIDER such foolish & stupid behavior.  I've just listened to "Master of Puppets" enough times to fuel my creativity.  Also, this song was originally written as, more or less, my take on "Enter Sandman".  I like these lyrics MUCH better because of their deeper metaphoric meaning.
